Terms and conditions for Maternity Leave Pass - 4 Weeks Unlimited
FIT4MOM SANTA MONICA MATERNITY PASS AGREEMENT:
This membership allows you to attend unlimited FIT4MOM Santa Monica classes (not including specialty programming such as Run Club and Body Well) for 4 weeks from date of purchase.
AUTHORIZATION FOR PAYMENT:
I authorize FIT4MOM Santa Monica to initiate a one time payment of $125 for this 4 week plan.
REFUND/HOLD POLICY:
No refunds and no holds or extensions allowed on this pass. In case of a medical emergency, please provide a doctors note and we will place your plan on a no fee medical hold for a maximum of three months. If you attend class before your hold is over without advanced notice, your credit card on file will be charged the drop-in rate of $25/class. If you would like to change your return date from your hold, email johannakracke@fit4mom.com with a doctor's clearance note at least 48 hrs prior to your return.
