Terms and conditions for Unlimited Monthly Membership
FIT4MOM Santa Monica MONTHLY MEMBERSHIP AGREEMENT:
This contract is made between the client and FIT4MOM Santa Monica and is a month to month agreement (after the initial 3 month commitment) and may be terminated by either party with the proper advance notice.
There is an early cancel fee of $49 if cancelled before the 3 month commitment.
This membership allows you to attend unlimited FIT4MOM Santa Monica classes.
AUTHORIZATION FOR PAYMENT:
I authorize FIT4MOM Santa Monica to initiate a charge to the account entered into Pike13 for my dues. This authorization is to remain effective until I have cancelled my membership in writing to johannakracke@fit4mom.com. I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Santa Monica to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.
CANCELLATION POLICY:
To cancel this membership, the member must contact FIT4MOM Santa Monica at johannakracke@fit4mom.com at least 15 days prior to their next payment due date to request a cancellation. Cancellations will be confirmed in writing to the member once they have been processed. No refunds will be given for dues once it has been paid. If cancelled within the first 3 months, a $49 early cancellation fee will be assessed. This membership is not transferable to another person or anywhere outside the FIT4MOM Santa Monica franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged the registration fee of $79. Instructors are not authorized to cancel memberships.
HOLD POLICY:
Memberships can be placed on hold for up to 3 months, with a maximum of one hold per calendar year. To place a hold on this membership, the member must contact FIT4MOM Santa Monica in writing at johannakracke@fit4mom.com at least 15 business days prior to when you'd like to start the hold. Instructors are not authorized to confirm holds. There will be a $25 processing fee charged to your credit card on file for the hold period. Holds will be confirmed in writing to the member once they have been processed. Holds can start and end on any day of your billing cycle. Our system auto-prorates your membership upon release of your hold. There are no refunds on dues once payment has been paid. When a hold period is ended, the monthly auto payments will automatically resume on the designated return date. If you attend class before your hold is over without advanced notice, your credit card on file will be charged the drop-in rate of $25/class. If you would like to change your return date from your hold, email johannakracke@fit4mom.com at least 48 hrs prior to your return.
